Watch: Fighting GLP-1 Fatigue

Starting a metabolic weight loss journey is one of the most powerful decisions you can make to reclaim your health, but it often comes with a hidden cost: systemic fatigue. In my personal case study, I started at 258 pounds in November 2025. Today, I am holding steady at 212 pounds—a total fat loss of 46 pounds. While my metabolic vitals are excellent and my physical structure (including my back) is feeling better than ever, maintaining cellular energy while in a caloric deficit on Tirzepatide (Mounjaro) required a highly strategic pivot in my daily protocol.

Rather than relying on heavy caffeine or stimulants that stress the nervous system, I designed an over-the-counter (OTC) supplement stack that supports mitochondrial function and catecholamine production from the ground up. This article breaks down the science behind my daily energy and joint recovery protocol.


The 5-Day Tirzepatide Cadence: Stabilizing Plasma Levels

One of the first adjustments I made to my protocol was shifting from a weekly (7-day) injection cadence to a 5-day cadence at my current dose of 7.5mg.

Tirzepatide has an elimination half-life of approximately 5 days. When you inject weekly, plasma levels of the drug experience significant peaks and troughs. By day 6 and 7, the concentration in your bloodstream drops, which often triggers:

  • The return of “food noise” (cravings and appetite spikes).
  • Subjective energy crashes as the metabolic signaling weakens.

By injecting 7.5mg every 5 days, I align the dosing frequency directly with the half-life of the compound. This creates a stable steady-state concentration in my bloodstream, eliminating late-cycle hunger spikes and providing constant, predictable glycemic and metabolic support.


The Mitochondrial Stack: Dual-Stage Energy Support

Delayed gastric emptying from GLP-1 receptor agonists can slow nutrient delivery, leading to post-meal lethargy and muscle fatigue. To bypass this, we must optimize the cell’s powerhouses: the mitochondria. My stack uses a dual-stage approach combining L-Carnitine and CoQ10.

1. L-Carnitine L-Tartrate (2,000mg)

L-Carnitine is responsible for transporting long-chain fatty acids across the inner mitochondrial membrane into the matrix. This is the rate-limiting step of beta-oxidation (fat burning). Without enough carnitine, fatty acids cannot enter the cellular furnace to be converted into energy.

To ensure muscular absorption, oral L-Carnitine requires an insulin spike to activate the sodium-dependent organic cation transporter (OCTN2). Therefore, I take my 2,000mg dose in the morning alongside a protein source, rather than fasted.

2. Coenzyme Q10 (CoQ10 – 100mg)

Once L-Carnitine delivers the fatty acids into the mitochondria, CoQ10 acts as the electron carrier in the respiratory chain, shuttling electrons to synthesize Adenosine Triphosphate (ATP)—the fundamental energy currency of the body. Combining L-Carnitine (fuel delivery) and CoQ10 (energy conversion) acts as a highly efficient metabolic pump.


The Focus Stack: Preserving Neurological Drive

Active weight loss and caloric restriction can drain catecholamines, leading to brain fog and a lack of motivation. Some GLP-1 users experience a feeling of “anhedonia” or emotional flattening.

To preserve focus without jittery stimulants, I supplement pre-workout with 1,500mg of L-Tyrosine. L-Tyrosine is the direct amino acid precursor to L-DOPA, dopamine, and norepinephrine. By supplementing with Tyrosine, I ensure my brain has the raw materials it needs to synthesize these drive-enhancing neurotransmitters, maintaining mental clarity and performance at work and in the gym.


Muscle Sparing & Sarcopenia Prevention

A rapid weight drop carries the severe risk of muscle catabolism (sarcopenia), which ultimately destroys your metabolic rate and leaves you “skinny fat.”

To lock in my lean mass, I anchor my morning routine with:

  • 1 Scoop of Whey Protein: Providing rapid-absorbing essential amino acids.
  • 5 Grams of Creatine Monohydrate: Drawing intracellular water to keep muscle cells hydrated and signaling anabolism, while fueling the phosphocreatine energy shuttle for rapid ATP regeneration during intense lifting.

Simplifying Joint Recovery: The Pituitary Reset

For joint healing, I have transitioned completely OFF the Wolverine peptide stack (BPC-157 and TB-500). Instead of forcing healing through continuous injections, I am allowing my sore shoulder to recover naturally through structured rest and taking time off from training.

Continuous stimulation of growth hormone pathways via peptides can deplete pituitary zinc stores over time. Growth hormone is stored in zinc-bound crystals, and depletion leads to peripheral growth hormone resistance and blunted IGF-1 levels. Stepping back and simplifying my stack to high-quality OTC supplements allows my pituitary receptors to desensitize and restore their mineral balance naturally.
